
Advanced Care Planning

Advanced Care Planning is the process by which a person develops and shares a plan that details the care they want should they be unable to express themselves at the time when a decision needs to be made about their care. Advanced care plans can be developed by anyone at any time. Unfortunately, many people don't think to do this before something forces them to do so.
A Death Doula can help forge, organize, and modify an advanced care plan. I help people express and record their values and priorities about their quality of life or comfort should they be unable to make decisions for themselves. The development of an advanced care plan can include conversations and decisions about:

if and when to hospitalize

if and when to resuscitate (CPR, medical ventilation, DNR)

choosing a Substitute Decision Maker (SDM) or Power of Attorney (POA), Will/Estate development

medical treatments (e.g. feeding tubes, IV, dialysis, etc.)

disposition of the body (burial, cremation, aquamation, etc.)

funeral/celebration of life arrangements (if any)

where death will take place (hospital, hospice, home)

what comforts will be available during the dying process (e.g. music, friends/family around, scents, readings, etc.)
